Wow, thanks! This goal of this initial questionnaire is to help us conduct one-on-one interviews with the actual clinic employees. However, we will be conducting patient interviews/surveys very soon.

These clinic employee interviews are a chance for employees to voice their ideas and concerns to our corporate marketing department. Their answers will shape how we develop our future marketing efforts, what events we throw, how we engage patients on an emotional level, how we upsell retail products, etc.

We're getting 30 minutes with each of the main clinic personnel: clinic managers, practitioners, account managers, and weight counselors. I'm looking to create an aggregated list of questions so I can segment question by job role.

What's your experience in creating surveys? It's too late for this questionnaire (due tomorrow), but I'm not above using contract resources for the patient questionnaire down the road.
